ACADEMY ELEMENTARY SCHOOL (1st-4th GRADES)
ENROLLMENT PROCEDURE
The primary focus at SAGE
Academy is to nurture children's natural inquisitiveness,
develop their self-confidence in their intelligence
and in their ability to succeed, and teach them
to be responsible and independent learners. To
this end we seek to enroll students
of sound character and educational promise who
will provide a diverse population in talent, background,
and experience.
If you wish to apply for admission
to SAGE Academy's Elementary School for the 2010-2011
school year, the application process is as follows:
1. Please fill out an application

Submit this form along with a $100.00 non-refundable
application fee to: SAGE Academy 5901 Lindley
Ave., Tarzana CA 91356
2. Submit a Confidential Evaluation
form and a stamped return envelope to your child's
current classroom teacher. Please submit the Request
for Pupil Records form to the office manager of
your current school. Please note that these
forms must be mailed directly from your child's
school to SAGE Academy.
5. Your child's space at SAGE
Academy's Elementary School (for grades 1st
- 4th) will be secured upon receipt of the admission's
application and the non-refundable deposit of
one month's payment. Additional materials will
be mailed to you in July.
